236.	236. / 239 PGT Technique This is the Cell stage of the embryos at time of the PGT biopsy event. More info: 236. Polar Body Biopsy: Polar bodies are the by-products of egg division. These cells do not serve any role for the egg or embryo and will naturally degrade; however, they can be removed and tested to determine the genetic status of the egg. Polar body testing only tests for the maternal genetic contribution to the embryo. Polar body biopsy occurs at the Day 0 and/or Day 1 stage. Both polar bodies must be removed and tested in order to make an accurate diagnosis. 237. Blastomere (Cleavage Stage) Biopsy: Blastomeres are cells from the Day 3 cleavage stage embryo, when approximately 6-8 cells are present. Each blastomere contains both maternal and paternal genetic information. Blastomere biopsy is the most common method of PGT, and involves the removal of one of these cells at the Day 3 stage. Since chromosomal mosaicism is highest at the Day 3 stage, blastomere testing may not always be representative of the genetic content of the embryo. 238. Blastocyst (Trophectoderm) Biopsy: A blastocyst is a Day 5/6 embryo which contains two cell types – the inner cell mass, which eventually develops into fetal tissues, and the trophectoderm, which gives rise to the developing placenta and other “extra-embryonic” tissues. Blastocyst biopsy involves the removal of a small number of trophectoderm cells for genetic testing. Testing the embryo at this stage has the advantage of reduced chromosomal mosaicism as well as improved DNA amplification, and therefore, improved test accuracy. 239. Array CGH Array Comparative Genome Hybridization is a molecular cytogenetic technique for the detection of chromosomal copy number changes on a genome wide and high-resolution scale. Array CGH compares the patient's genome against a reference genome and identifies differences between the two genomes, and hence locates regions of genomic imbalances in the patient, utilizing the same principles of competitive fluorescence in situ hybridization as traditional CGH. 241. Metaphase CGH Metaphase Comparative Genomic Hybridization (mCGH) allows the detection of chromosomal aberrations across the entire genome with a maximal resolution of >10 Mb. In its original form, DNA is isolated and fragmented from an experimental sample and a control sample. Both samples are then labeled with two different fluorescent colors, usually green and red. The DNA samples are mixed and the generated probes compete for hybridization on a normal chromosome. If both samples are identical, a mixed yellow/orange color is seen everywhere. A red signal indicates an overrepresentation of the red-labeled experimental DNA, the result of gene duplication. A green signal, on the other hand, indicates a gene loss. This form of CGH requires metaphase chromosomes, which reduces the resolution. 242. PCR Polymerase chain reaction is a method widely used in molecular biology to make many copies of a specific DNA segment. Using PCR, copies of DNA sequences are exponentially amplified to generate thousands to millions of more copies of that particular DNA segment. PCR is now a common and often indispensable technique used in medical laboratory and clinical laboratory research for a broad variety of applications including biomedical research and criminal forensics 243. NGS Next-Generation Squencing (NGS), also known as high-throughput sequencing, is the catch-all term used to describe a number of different modern sequencing technologies. NGS platforms perform sequencing of millions of small fragments of DNA in parallel. Bioinformatics analyses are used to piece together these fragments by mapping the individual reads to the human reference genome. Each of the three billion bases in the human genome is sequenced multiple times, providing high depth to deliver accurate data and an insight into unexpected DNA variation. 244. SNP Microarray Single Nucleotide Polymorphism SNPs (pronounced snips) are pairs of single nucleotides (A, T, C, or G) in genomic DNA that are highly variable within a given species. This is a type of DNA microarray which is used to detect polymorphisms within a population. A single nucleotide polymorphism (SNP), a variation at a single site in DNA, is the most frequent type of variation in the genome. High density SNP arrays help scientists identify patterns of allelic imbalance. In the context of PGS, the SNPs evaluated are generally in non-exon coding segments of the genome. SNP microarrays in PGS typically evaluate approximately 300,000 SNPs spaced throughout the genome. SNP arrays provide a genotype (AA, AB, or BB) for each sample analyzed and compare these results to a human hap map reference genome. These arrays can identify whole chromosome aneuploidy and can also identify approximately 250 common structural chromosome aberrations throughout the genome. 245. FISH Fluorescence in situ hybridization is a molecular cytogenetic technique that uses fluorescent probes that bind to only those parts of a nucleic acid sequence with a high degree of sequence complementarity. It was developed by biomedical researchers in the early 1980s to detect and localize the presence or absence of specific sequences on chromosomes. Fluorescence microscopy can be used to find out where the fluorescent probe is bound to the chromosomes. FISH is often used for finding specific features in DNA for use in genetic counseling, medicine, and species identification. FISH can also be used to detect and localize specific RNA targets (mRNA, lncRNA and miRNA) in cells, circulating tumor cells, and tissue samples. In this context, it can help define the spatial-temporal patterns of gene expression within cells and tissues. May be valued when PGD entered is “All Embryos” or “Some Embryos”.
