Surrogacy arrangement treatment types in IDEAS

The treatment type configuration controls what types of forms are available for you to complete in IDEAS PRISM Portal. You must be consistent in the treatment type you use for the different cycles you create in a surrogacy arrangement.

Clinics use many different names for the cycle types involved in surrogacy and at deployment we tried to determine what type was used for what and configure appropriately. However this configuration may need changed to suit your purposes.

Please contact support@mellowoodmedical.com, stating PRISM somewhere in the title if you feel that the correct forms are not displaying for these cycles and we can review the configuration

What should be reported in surrogacy cycles, and from which records?

From HFEA;

The embryo creation and use (donation) is reported under the intended mother, and both FET (including thaw) or Fresh embryo transfer is reported under the surrogate.

For an FET the thaw and transfer are reported under the surrogate.

If embryo biopsy is part of these cycles, this should be reported on the intended parent forms, in the same way that it would be for a patient using their own embryos

  • IMPORTANT POINTS

  • When embryos are thawed , they should ALWAYS be thawed in the record of the person having the embryo transfer ie. the surrogate, not transferred in the intended parent record and then donated.

  • Frozen embryos should always be ‘claimed’ in the cryomodule for the GC before adding lab events to the GC thaw cycle

  • outcomes - pregnancy tests and gestational scans should always be on the treatment cycle of the embryo transfer, NOT the intended parent

Male:male couples freezing embryos for surrogacy - purpose should always be set to ‘donation for use by others’ in core IDEAS , even if the embryos are brought in from another clinic

SCENARIOS

Intended parents creating and storing embryos with own eggs and partner or donor sperm and transferring to a surrogate in a THAW cycle

Storage cycle - intended parent record

  • IDEAS - Embryos should be created and frozen in the intended parent treatment cycle,

    • If known at the time, the embryos should be stored with purpose ‘stored for donation by others’. If not known at the time you can store for own use and change this later.

  • IDEAS - Treatment type should be ‘Storage cycle for surrogacy or something similar’

    • PRISM - Registered as ‘Intended Mother’. If she has previously been registered as a patient you will need to submit a change of role.

    • If a male:male couple then they are registered as ‘Intended father’. males cannot be registered as patients as they are not undergoing licensed treatment themselves

    • PRISM - submit Treatment cycle form

    • PRISM - Report on Embryo Creation and Use form ensuring the embryo freeze is reported as ‘for donation’


Transfer cycle - Surrogate/Gestational Carrier record

  • PRISM - Surrogate should be registered as such

  • IDEAS - if the embryos have been originally stored as ‘for patient use’ you should change the purpose to ‘donation for use by others’ using the ‘claim’ button in the cryomodule

 

  • IDEAS - in the cryomodule, claim the embryos that are to be thawed in the GC cycle for the GC (the recipient) 'Claiming' frozen gametes or embryos for recipient use

  • IDEAS - Create a treatment cycle - should be 'FET (surrogacy) or similar

  • IDEAS

    • Identify the egg source , and cycle source of the frozen embryo to be thawed (if you can’t find the source cycle on the drop down you may have to ‘unlock’ it)

    • tick gestational carrier on the Gametes tab of the Treatment Plan

  • IDEAS - Thaw from the intended parent donation cycle using ‘get eggs/embryos’ and choosing ‘thaw from donor cycle’

  • The embryo(s) should be available to select and thaw. Enter the subsequent lab events

  • PRISM - submit treatment cycle form

    • PRISM - Report to HFEA on Thaw Transfer and Refreeze form - you will have to search inventory with the egg source i.e. intended mother details to find the embryos

Intended parents creating embryos with own eggs and partner or donor sperm and transferring to a surrogate in a FRESH cycle

Fresh cycle - intended parent record

  • IDEAS - Use an ‘Embryo creation - intended parent’ cycle type or similar, in the intended parent record, to create and donate embryos, using a donation lab event

  • PRISM - Register Female as ‘Intended Mother’. If she has previously been registered as a patient you will need to submit a change of role.

  • If a male:male couple then they are registered as ‘Intended father’. males cannot be registered as patients as they are not undergoing licensed treatment themselves

  • PRISM - submit treatment cycle form

  • PRISM - submit Embryo Creation and Use form

    • you will be presented with a section to document how many fresh embryos are being donated


Transfer cycle - Surrogate/Gestational carrier record

  • IDEAS - Treatment cycle should be ‘Surrogate transfer cycle’ or similar

  • IDEAS - tick gestational carrier on the Gametes tab of the Treatment Plan

  • IDEAS - receive the embryos from the intended parent donation cycle using ‘get eggs/embryos’ and selecting ‘From donor cycle’

  • The embryo(s) should be available to select. Enter the subsequent lab events

  • PRISM - Register as surrogate

    • PRISM - submit treatment cycle form - you won’t need to select any features

    • PRISM - Report to HFEA on Embryo Recipient form

Intended parents using frozen/thawed donor eggs to create and store embryos in a FRESH CYCLE and transfer to gestational carrier in a THAW cycle

Egg collection & freeze - Egg donor record

  • IDEAS - use a treatment type ‘egg storage for donation’ treatment cycle - should involve collecting eggs and freezing with the purpose ‘donation for use by others'’

    • PRISM - Register as egg donor

    • PRISM - Submit ‘treatment cycle’ form

    • PRISM - Submit ‘egg collection form’ - ensure 'number of eggs donated/for use in surrogacy’ has been completed

  • IDEAS - Complete Treatment & Cycle

NOTE - if the eggs are being imported from another clinic that should be reported on a Gamete IN form and the purpose set as ‘donation for use by others’


Receiving frozen oocytes and embryo creation - Intended parent record (this may be male or female)

Note - you may need to contact Mellowood to make a change to allow you to create a treatment cycle on a male record

  • IDEAS - claim eggs in storage

  • IDEAS - create an egg thaw cycle (or equivalent in your IDEAS), identifying the oocyte source as donor

    • PRISM - register as intended parent

    • If a male:male couple then they are registered as ‘Intended father’. males cannot be registered as patients as they are not undergoing licensed treatment themselves

  • IDEAS - use ‘get eggs/embryos' button and select the ‘thaw from donor cycle’ option to thaw the eggs, inseminate and freeze embryos - purpose of ‘donation for use by others’

    • PRISM - Submit Treatment cycle Form

    • PRISM - submit ‘Egg thaw form’

    • PRISM - Submit Embryo Creation & Use form - reporting the insemination and embryo creation & storage for donation

  • IDEAS - Complete Treatment & Cycle


Transfer cycle - Surrogate/Gestational Carrier record

    • Identify the egg source , and cycle source of the frozen embryo to be thawed (if you can’t find the source cycle on the drop down you may have to ‘unlock’ it)

    • tick gestational carrier on the Gametes tab of the Treatment Plan

Intended parents using fresh donor eggs to create and store embryos in a FRESH CYCLE and transfer to gestational carrier in a THAW cycle

Egg collection - Egg donor record

  • IDEAS - use a treatment type ‘egg donation’ should involve collecting eggs and donating

    • PRISM - Register as egg donor

    • PRISM - Submit ‘treatment cycle’ form

    • PRISM - Submit ‘egg collection form’ - ensure the ‘number of fresh eggs donated/for use in surrogacy ’ has been completed

  • IDEAS - Complete Treatment & Cycle


Receiving oocytes and embryo creation - Intended parent record (this may be a female or a male record

Note - you may need to contact Mellowood to make a change to allow you to create a treatment cycle on a male record

  • IDEAS - create an egg recipient cycle (or equivalent in your IDEAS), identifying the oocyte source as donor

    • PRISM - register as intended parent

    • If a male:male couple then they are registered as ‘Intended father’. males cannot be registered as patients as they are not undergoing licensed treatment themselves

  • IDEAS - use ‘get eggs/embryos' button and select the ‘from donor cycle’ option to receive the eggs, inseminate and freeze embryos - purpose of ‘donation for use by others’

    • PRISM - Submit Treatment cycle Form

    • PRISM - Submit Embryo Creation & Use form - reporting the receipt of eggs, insemination and embryo creation & storage with purpose 'donation for use in surrogacy cycle

  • IDEAS - Complete Treatment & Cycle


Transfer cycle - Surrogate/Gestational Carrier record

  • PRISM - Surrogate should be registered as such

  • IDEAS - if the embryos have been originally stored as ‘for patient use’ you should change the purpose to ‘donation for use by others’

 

  • IDEAS - in the cryomodule, claim the embryos that are to be thawed in the GC cycle for the GC (the recipient) 'Claiming' frozen gametes or embryos for recipient use

  • IDEAS - Create a treatment cycle - should be 'FET (surrogacy) or similar

  • IDEAS

    • Identify the egg source , and cycle source of the frozen embryo to be thawed (if you can’t find the source cycle on the drop down you may have to ‘unlock’ it)

    • tick gestational carrier on the Gametes tab of the Treatment Plan

  • IDEAS - Thaw from the intended parent donation cycle using ‘get eggs/embryos’ and choosing ‘thaw from donor cycle’

  • The embryo(s) should be available to select and thaw. Enter the subsequent lab events

  • PRISM - submit treatment cycle form

    • PRISM - Report to HFEA on Thaw Transfer and Refreeze form - you will have to search inventory with the egg source (remember this will be the egg donor details not the intended mother) details to find the embryos

M:M Intended parents bringing in embryos created with their sperm and donor eggs from another clinic -and transferring to a surrogate in a THAW cycle

Gamete/Embryo IN cycle - Intended parent record (this may be a female or a male record

Note - you may need to contact Mellowood to make a change to allow you to create a treatment cycle on a male record

  • IDEAS - Create a Gamete in Cycle as you do any other, in the record of the member of the couple who is the gamete source for the embryos (if appropriate) - How to - Gamete Movement IN .

    • PRISM - register as intended parent. If a male:male couple then they are registered as ‘Intended father’. males cannot be registered as patients as they are not undergoing licensed treatment themselves

    • PRISM - Submit Gamete Movement IN form

      • The gamete source will be ‘donor eggs and intended father sperm’

        • You will need to know the reg number of the intended father AND the egg donor at the supplying clinic

Transfer cycle - Surrogate/Gestational Carrier record

  • PRISM - Surrogate should be registered as such

  • IDEAS - if the embryos have been originally stored as ‘for patient use’ you should change the purpose to ‘donation for use by others’ in the cryomodule

 

  • IDEAS - in the cryomodule, claim the embryos that are to be thawed in the GC cycle for the GC (the recipient) 'Claiming' frozen gametes or embryos for recipient use

  • IDEAS - Create a treatment cycle - should be 'FET (surrogacy) or similar

  • IDEAS

    • Identify the embryo source , and cycle source of the frozen embryo to be thawed (if you can’t find the source cycle on the drop down you may have to ‘unlock’ it)

    • tick gestational carrier on the Gametes tab of the Treatment Plan

  • IDEAS - Thaw from the intended parent donation cycle using ‘get eggs/embryos’ and choosing ‘thaw from donor cycle’

  • The embryo(s) should be available to select and thaw. Enter the subsequent lab events

  • PRISM - submit treatment cycle form

    • PRISM - Report to HFEA on Thaw Transfer and Refreeze form - you will have to search inventory with the egg donor registration number and reg centre (it won’t be your centre) details to find the embryos
