Shared Motherhood and PRISM

We know that HFEA has refined their definitions and a shared motherhood cycle, where one partner creates embryos with her eggs and then the other partner has the embryo transfer and hopefully carries the pregnancy , is no longer referred to as a donation cycle, and the participants are not known as a donor and a recipient. However the events the you need to use in IDEAS are still referred to as donations. This does to reflect in HFEA, as the participants are registered with appropriate roles
From HFEA;
In PRISM, for female:female couples the egg provider is registered as the “patient” (P1) and the partner is registered as normal “partner” (P2).
Stimulation, egg collection and embryo creation and use (including storage) are recorded under the patient (P1).
To record an FET or fresh embryo transfer under P2 (partner). The clinic will first need to swap the role from “partner” to “patient".

SCENARIOS
Fresh embryo donation - shared motherhood
REGISTRATION
PRISM - Register P1 - egg source - as ‘Patient’
PRISM - Register P2 as ‘Partner’
P1 - Fresh embryo donation
IDEAS - Create Embryo Donation or IVF treatment cycle
PRISM - Submit Tx cycle form
IDEAS – Collect eggs – laboratory details
PRISM - Submit Egg Collection form
IDEAS – Create embryos using P1 eggs and donor sperm - and donate these embryos using the ‘donate’ lab event and selecting the embryos by clicking the grey tick. It will turn green (example below)
Any remaining embryos can be frozen or discarded in the donor cycle
PRISM – Submit Embryo Creation and Use
Gamete source will be ‘Patient eggs and donor sperm’
Fresh donated section should have the number donated in field ‘No. of embryo's donated fresh / For use in surrogacy’ - see example screenshot below
P2 - Recipient of fresh donated embryos
PRISM – Edit the registration form and change the role of P2 to ‘Patient’
PRISM - Edit the registration form and change the role of P1 to ‘Partner’
IDEAS - Create Embryo Recipient treatment cycle
Gamete source - indicate the ovum source, and also the treatment cycle that the embryos were created in
PRISM - Submit Tx cycle form making sure that the role is selected as patient and choose the treatment type as indicated below
IDEAS – Laboratory details
Click ‘get eggs/embryos’ and select ‘from donor cycle’ option
Select the donated embryos from P1
Transfer, Store or Discard these embryos
PRISM - Submit Embryo Recipient form
Click ‘collect’
Enter the registration number of P1
Click search, which should provide the inventory of donated embryos from P1
click ‘thaw’ for appropriate embryos, complete the form and save

IDEAS - Create a cycle to create the embryos
PRISM - Submit Tx cycle form

IDEAS – Create embryos using P1 eggs and donor sperm
Store these embryos - any embryos that are definitely known to be donated to P2, store with the purpose ‘donation for use by others’
PRISM – Submit Embryo Creation and Use - Gamete source will be ‘Patient eggs and donor sperm’
What do I do if the embryos are initially created for the patient use and then she decides to donate to P2 for thaw and transfer?
change the purpose in the cryomodule to ‘cryo for donation’
add a ‘donation’ lab event to the original embryo creation cycle, for the embryos to be donated - ONLY DO THIS WHEN YOU ARE PREPARING TO THAW FOR TRANSFER TO THE PARTNER NOT BEFORE
Amend the embryo creation form freeze details to indicate that the relevant number have been stored for donation
P2 - Recipient of frozen donated embryos
IDEAS - if the embryos have been originally stored as ‘for patient use’ you should change the purpose to ‘donation for use by others’ OR ‘cryo for partner’s use’
IDEAS - in the cryomodule, claim the embryos that are to be thawed in the P2 cycle for P2 'Claiming' frozen gametes or embryos for recipient use
PRISM – Edit the registration form and change the role of P2 to ‘Patient’ and the role of P1 to ‘partner’
IDEAS - Create Frozen Embryo Transfer recipient treatment cycle NO DONATION CYCLE IS REQUIRED IN THE RECORD OF P1
IDEAS – Laboratory details - Click ‘Get Eggs and Embryos’ and select ‘Thaw from previous or donor cycle’ ;
Select the donated stored embryos from P1
Transfer, Store or Discard these embryos in IDEAS lab events.
PRISM - Submit Tx cycle form - with role ‘patient’
PRISM - Submit Frozen Embryo Transfer form - Select Gamete Source = ‘Partner’ which should provide the inventory of donated frozen embryos from P1
Select the embryos, finish complete the form and save;
IN PRISM ENSURE THAT A FROZEN EMBRYO TRANSFER THAW FORM IS AVAILABLE, NOT A FRESH EMBRYO RECIPIENT FORM - IF NOT CONTACT SUPPORT AT MELLOWOOD AS YOUR CONFIGURATION MAY BE INCORRECT

Create Egg Donation treatment cycle
Laboratory Details folder
Collect egg 'using ‘get eggs/embryos’ button
Donate these eggs by setting the fate to ‘donate to partner’ in the egg collection lab event
Complete the treatment and cycle

Gametes tab - Identify donor, the donation treatment cycle in the Gametes tab
Laboratory details
Use ‘get egg/embryo’ and use the ‘from donor cycle’ option;
this will allow you to ''select and receive’ the selected eggs from the donation cycle identified in the gametes tab
Add the appropriate lab events up until Embryo Transfer

IDEAS - Create Frozen Egg recipient treatment cycle type NO DONATION CYCLE IS REQUIRED IN THE RECORD OF P1, the donation event happens in the cycle that the eggs were stored in
IDEAS - Gametes tab - Identify the cycle the eggs are being donated from in the Gamete Source tab
Laboratory details
Click ‘Get Eggs and Embryos’ and select ‘Thaw from previous or donor cycle’;
Select the donated stored eggs from P1
Use the appropriate lab events to create, transfer, store or discard these embryos
