Early Outcome / Pregnancy Outcome Form - Incorrect number of fetal hearts

There has been reported cases where a HFEA validation error (illustrated) occurs when attempting to submit a pregnancy outcome.

There is a specific workflow to report Early Outcomes and Outcomes

I get this error - My patient is CLINICALLY Pregnant (fetal heart seen) after a pregnancy test

You will need to report an Early Outcome with at least 1 fetal heart and at least 1 gestational sac.

Make sure that the number of fetal hearts on the Pregnancy Outcome MATCH the number of fetal hearts on the Early Outcome

You only need to report an Early Outcome !!

If you are still seeing the Pregnancy Outcome Form

if it is green and marked as DONE - delete it from IDEAS PRISM Portal

If it’s still red and unsubmitted, you can remove it by clearing the Estimated Delivery date and clinical pregnancy date in IDEAS

Which should then only display the Early Outcome as the last form.

If your patient was not clinically pregnant, but you incorrectly submitted a pregnancy outcome form in error you will get this error.

If you can see the outcome form in IDEAS PRISM Portal with an either green or blue border
- Delete the Pregnancy Outcome Form in IDEAS PRISM
Remove the Estimated Delivery Date & Clinical Pregnancy date in IDEAS

You have probably submitted the form by accident, and then deleted the clinical and delivery dates in IDEAS , without deleting the PRISM form first .
