This chapter explains in what IDEAS fields retrieves the information to fill in the CARTR form.

The CARTR form can be found in the last tab of the  Treatment cycles.

The CARTR form has 5 sections:

  • Demographic slice

  • The toolbar

  • The validation box

  • Tooltip box

  • Tabs

General remarks

When this tab is opened not every field will be visible because this field might be conditional and the field is mandatory in certain situations.
When the  Collect is clicked only the relevant fields are visible.

Behind every section and field you see two lock icons.

This means the field is unlocked and can be changed if the   Collect button is clicked. -

This means the field is locked and can NOT be changed if the Collect button is clicked.

When the  icon of a section is clicked it will lock every field in that section. Theicon will unlock it.
The icon does NOT mean that the field can’t be altered.

There are several fields that have a list with options. In these fields more than one answers are possible.
When selected the option will move to the top, so they are directly visible.

The toolbar

On the top you can see the patient name with her dob and cycle number and start date.

 

The toolbar has the following buttons:

- This will save and close the form

- This will save the form

- This will undo the last “save”

- This will close the form without saving

- Collects the last information from IDEAS and fills in the form

  • Collect all                               - This will collect all information for all the tabs

  • Collect outcome data only - This will collect only the data for ”Outcome” tab.
                                                        (i.e. if Cycle  development is already signed off

  • Disable collecting - Collecting is not possible.

  • Cycle is closed - The cycle is signed off.

- This will clear the form. 

Demographic slice

The demographics slice is the same as the demographic slice of the patient file.
The Cycle ID and the Cycle Start Date are also displayed.

 

The status might differ from the demographic slice of IDEAS because the demographic slice of the CARTR form is at the time of the cycle and the demographic slice of IDEAS shows the current info.

Validation box

On the right of the form you can see the validation box.
When a field is not correctly filled in, a message will appear here telling you what is wrong with the data entry.

Tooltip box

When you hover over a field, information related to the field and requirements will appear in this section.

It also shows where in IDEAS this information should be entered.

The CARTR form has 9 tabs as listed below. Details about the fields on each tab can be viewed in the individual pages.
