Oocyte collection section

In this section, 74. Oocyte collection date, 78. Oocyte Origin, and 82. # Oocytes Retrieved / Thawed are required.
In an IVF or IVM cycle, the three fields for entering information about thawed oocytes are to be used in the rare situation that a fresh oocyte retrieval is attempted with no or insufficient oocytes retrieved, so some frozen oocytes are thawed.
If 78. Oocyte Origin includes donor oocytes, fill in information about the donor in the next two boxes (79. Oocyte Donor Status is required).
If any oocytes are cryopreserved, Oocyte freeze ID, date and method need to be filled in.

Oocyte collection date
Date of oocyte collection should be < 60 days after 20. CycleStart Date.

This needs to be filled in the Egg collection event of the Laboratory details

Should be <60 days after 20. Cycle start date (ART0080)

75.

Oocyte thaw date
Date of oocyte thawing/warming.

Mandatory for FOIVF cycles and IVF, IVM and OB cycles

 

76.

Freeze ID # 1 of thawed oocytes
Unique code originally assigned to the oocyte cohort at freezing.

Optional field

 

77.

Freeze ID # 2 of thawed oocytes
For frozen oocytes received from another clinic, code given by the receiving clinic to that cohort of oocytes.
This is an internal number created by IDEAS.

Optional field

 

78.

Oocyte origin
Origin of oocytes used to create embryos or for banking.
Choose partner oocytes when same-sex female partner provides oocytes for same-sex partner to be the embryo recipient.

79.

Oocyte donor ID
Unique code to identify the oocyte donor.

Only applicable if 76.Oocyte Origin (ART0370) = 'Fresh donor oocytes' or 'Frozen donor oocytes'

 

80.

Oocyte provider source
Unique name of commercial oocyte bank for oocytes sourced from commercial egg bank or chart number of donor patient (if donated at same clinic).

Optional field

This needs to be filled in manually.

81.

Donor / provider status
Type of donation.For same-sex female partner oocyte provider, choose non-anonymous.
If oocyte provider intends to keep some oocytes for own use, choose non-anonymous or anonymous oocyte sharing, whichever applies.

Only applicable if 76.Oocyte Origin (ART0370) = 'Fresh donor oocytes' or 'Frozen donor oocytes'

Number oocytes retrieved
Number of cumulus-oocyte complexes (COCs) collected and/or number of oocytes thawed.

Range: 0 - 200
UI Warning: Value should be between 0 and 55

This needs to be filled in the Egg collection event.

 

83.

# oocytes cryo preserved
Number of oocytes frozen or vitrified in an oocyte banking cycle or an IVF cycle e.g. no sperm or very few sperm were available.

Mandatory if 82. Number Oocytes Retrieved or Thawed (ART0410) > 0


If none, set to 0

84.

Oocyte freeze ID
Unique code to identify that cohort of frozen oocytes.

Only applicable if 83. # of oocytes cryopreserved (ART0420) > 0

 

85.

Oocyte freeze date
Date of oocyte cryopreservation.

Mandatory if 83. # of oocytes cryopreserved (ART0420) > 0

 

86.

Oocyte cryo method
Type of method used to cryopreserve the oocytes.

Mandatory if 83. # of oocytes cryopreserved (ART0420) > 0

Slow freezing
Vitrification
Mixed

Sperm collection section

This section only needs to be filled in if insemination is done.
If the Sperm Origin is donor sperm, the sperm donor ID and status needs to be filled.
The semen/sperm parameters only need to be filled in if fresh partner sperm from semen is used.

This section is only visible when the cycle type is:

  • FOIVF

  • FET

  • IVF

  • IVM

Sperm donor status
Type of donation.

Only applicable if 87. Sperm Origin (ART0440) =

  • Frozen donor sperm (SPM003) or

  • Fresh donor sperm (SPM007)

  • Anonymous              SDS001

  • Directed donation      SDS002

Click on the button (Blue). Type of relationship.

 

91.

Age of sperm provider
Age of sperm provider at creation of embryos

Only applicable if 98. # oocytes inseminated/injected (ART0530) > 0
Range: 15 - 80

Do not enter if more than one sperm provider is used in this cycle or if sperm from more than one collection date is used in this cycle (e.g. fresh and frozen sperm used)

92.

Sperm source
Source of collected sperm (for partner if both partner and donor sperm used).
Ideas collects this information from the   Semen analysis of the sample used for the cycle.

Mandatory if 98. # oocytes inseminated/injected (ART0530) > 0
Cannot be 'Fresh own sperm' or 'Frozen own sperm' if 78. Oocyte Origin (ART0370) = '…Own Oocytes'

Range: 0.1 - 15 mL

If value < 1.3 mL, then 42. Reasons for Treatment Cycle (ART0150) will auto-populate to include 'Male Factor (inferred)' on SART site

94.

Sperm count
Sperm concentration in fresh (neat, pre-wash) semen sample

If sperm count is extremely low, 92. Sperm Too Low to Count will be ticked and 91. Sperm Count and 93. Sperm Motility are not available

Only applicable if 87. Sperm Origin (ART0440) = 'Fresh donor sperm', 'Fresh partner sperm' or 'Fresh own sperm' and 92. Sperm Source (ART0480) = 'Semen' and
95. Sperm too low to count (ART0505) <> Y.

If any value < 15 million, then r 42. Reasons for Treatment Cycle (ART0150) will auto-populate to include 'Male Factor (inferred)' on SART site

Applicable if:

87. Sperm Origin:

  • Fresh partner sperm

  • Fresh own sperm

  • Fresh own/partner sperm

92. SpermSource = Semen
95. SpermTooLowCount <> Y.

95.

Sperm to low to count.
Too few sperm to make an accurate count (Less than 0.1 mL).

When above is filled in it will show “No” otherwise it will be blank, and needs to be checked by user.

Only applicable if 87.Sperm Origin (ART0440) = 'Fresh donor sperm', 'Fresh partner sperm' or 'Fresh own sperm' and 92. Sperm Source (ART0480) = 'Semen' and Sperm too low to count (ART0505) <> Y.

96.

Sperm motility
Percentage of motile sperm in fresh (neat, pre-wash) semen sample.
Only applicable for fresh partner semen.

Only applicable if 87.Sperm Origin (ART0440) = 'Fresh donor sperm', 'Fresh partner sperm' or 'Fresh own sperm' and 92. Sperm Source (ART0480) = 'Semen' and Sperm too low to count (ART0505) <> Y.

If any value < 40%, then 42.Reasons for Treatment Cycle (ART0150) will auto-populate to include 'Male Factor (inferred)'
