Checklists can be linked to a cycle, and if the events are not completed further data cannot be entered into the cycle, or they can be created as an Application , which is stand alone and cycle events are not dependent.

Prerequisite;

Before using checklists in a patient record, you should have created your templates - for information on how to create/configure checklist templates follow this link Config. - Checklist templates

Adding a Checklist to a patient record

Checklists can be automatically included as part of the treatment plan , in a treatment module/plan, or can be added manually when required.

Automatically adding a checklist to a treatment cycle as part of a treatment plan

You can add a checklist to your treatment plan, most probably in a ‘pre treatment’ module. This means that when you create a treatment cycle and allocate the plan, the checklist will automatically appear in the Checklist folder, and all the requested items will need to be marked as completed before the cycle can proceed.

Manually adding a checklist to a treatment cycle

You may not always want to add a checklist to a treatment cycle, or you may have a selection of checklists to choose from , depending on the cycle type, and therefore want to choose and add the checklist manually , rather than it automatically be created as part of a plan.

  • There must be a treatment cycle created

  • Go to checklist folder and ensure that the appropriate cycle is selected in the drop down in the top right corner.

  • Click New, Select the template you require and click save .

  • You can then select the items you wish to request.

Note - unlike the checklist integrated into the treatment plan as described above, a manually added, uncomplete checklist will not stop the cycle from proceeding

Adding a checklist not associated with a treatment cycle

You may want to to add a checklist to a patient record , and it not be associated with a treatment cycle, e.g. a list of things that must be completed before the patient attends a face to face consultation.

You will need to create an ‘Application’

  • Select the Treatment Cycle folder

  • Click New and you should have the choice of ‘Treatment Cycle’, ‘Episode’ or ‘Application’

 

  • Choose Application

  • this will appear in the treatment cycle list with the prefix APP, rather than PRE or TX (which are true treatment cycles.

  • Go to checklist folder and add a checklist in the same manner as described above

 

 

You can see from the screenshot above that using an Application is a way to manage patients before they are accepted for treatment , and allows you to note some initial details about them and add them to a waiting list. If you do not have the option to choose application , you should enable this in the System Options System Options - Treatment Tab

Requesting items on the checklist

The creator of the checklist can either request that all items on the list be completed or that only some are requested.

On first opening of the checklist, in the case of ultrasounds and blood tests, if the item is already present in the patient record AND associated with the same cycle or application as the checklist, the results will show in the ‘Result from treatment plan’ column.

The user can then determine if they want to request another ultrasound or blood test.

Viewing the requested/planned items in the Treatment Cycle or Application

Once items are requested, they become planned items, and indicated as such in the ‘result from treatment plan’ column.

Planned/requested items appear in various places in the patient record, depending on their configuration in the template.

The configuration of the template determines where planned/requested items appear in the patient record

Using the above checklist as an example, the ‘type’ in the template determines where the items will be seen;

 

All planned items show in the plan details tab;

 

 

Planned/requested ART procedures & semen procedures types show in the laboratory details folder, under the planning tab;

 

 

Be aware the ART procedures requested on a checklist may conflict with planned items created for use in the LabPLan Module. Always review the planning details in the laboratory details folder when preparing for the lab events

 

Planned ultrasound type scans appear in the ultrasound folder

 

Other items may appear in the plan details areas, depending on other items such as the treatment plan associated with the cycle.

Example - Requesting an Ultrasound Scan

If you request an ultra sound scan on the checklist, a planned ultrasound event will show in the Ultrasound folder, associated with the particular cycle or application.

If there is already an ultrasound record associated with this Cycle or Application , the results will show in the result column. You can decide if you wish to request another by ticking request - this will result in a planned ultrasound record being added to the USS folder. If the existing scan is sufficient and you do not require to plan another ENSURE REQUEST IS NOT TICKED before you close the checklist.

If you have requested an ultrasound scan in the checklist;

The planned ultrasound scan will show in the ultrasound folder When the ultrasound scan is confirmed and data is entered, that data will then show back in the checklist for review. You will still need to manually complete the item in the checklist ;

 

NOTE - you may have an ultrasound scan on your checklist but have done a scan on that patient recently - be aware that if it is not linked to the same treatment cycle or application as the checklist, the result will not show in the ‘result from treatment plan’ column

Completing Requested items on the checklist

When the items are completed they are indicated as such, and the user who completed them is identified along with the date and time

If Progress notes type item is requested from the checklist and are submitted through the patient portal, they will be automatically linked to the treatment cycle (as long as it has a start date) and will be automatically marked as completed on the checklist when they are submitted

If a checklist is linked to a Treatment Cycle (rather than a stand alone Application) then the requested items will show in the Treatment Plan details, along with any drugs, ultrasounds and tests applied to the cycle using the Treatment Plan (if used).

Signing off Checklists

If you have IDEAS Version 7.0.13 or above you will not be able to sign it off unless all the requested items have ticked as completed;
