DD - Physicians/staff tab

Specify the patient physician information such as who referred the patient, the patient PCP or GP, from the pre-configred drop down menus.

If appropriate, specify the patient clinic information such as the staff physician, nurse, lab technician, embryologist etc. assigned to the patient.
